Case Management Referrals

Specialist Occupational Therapy within a wider rehabilitation plan

At Serenity Occupational Therapy, we work with case managers and rehabilitation professionals to provide specialist occupational therapy assessment, intervention and vocational rehabilitation for adults experiencing difficulties with everyday functioning.

Our approach is practical, person-centred and focused on helping people participate in the activities that matter to them.

How Occupational Therapy Can Help

We can support clients experiencing difficulties with:

  • Everyday activities and independence

  • Routine and structure

  • Fatigue and energy management

  • Cognitive and executive functioning

  • Sensory processing

  • Emotional regulation

  • Mental health and occupational functioning

  • Neurodivergence

  • Confidence and participation

  • Return to work

  • Maintaining employment

  • Work-life balance

  • Vocational rehabilitation

Our Approch

We start by understanding the person, their circumstances and what they need to be able to do through a full and comprehensive assessment.

We can then work collaboratively with the client and, where appropriate, the wider rehabilitation team to identify practical strategies and achievable goals.

Our occupational therapy intervention may include goal setting, graded activity, routine development, cognitive strategies, fatigue management, environmental adaptations, workplace strategies and vocational rehabilitation.

Working Alongside Case Managers

We understand that occupational therapy is often one part of a wider rehabilitation plan.

With appropriate consent, we can communicate with case managers and other professionals involved in the client's care to ensure our assessment and recommendations complement the wider rehabilitation programme.

We can provide:

Functional Occupational Therapy Assessments

A detailed assessment of occupational performance, functional difficulties, strengths, barriers and rehabilitation needs.

Occupational Therapy Assessment and Intervention

Assessment followed by individualised intervention to help the client make meaningful and sustainable changes.

Vocational Rehabilitation

Specialist support for clients whose health is affecting their ability to work, including return-to-work planning and workplace recommendations.
